I had struggled a bit with assessment in PSII, so I really wanted to make it a focus for PSIII. My Professional Inquiry Project was based on assessment and I made one of my goals for my Professional Growth Plan working on properly and appropriately assessing students as well. As I completed PSIII, I tried many new and different types of both formative and summative assessment, based on advice and guidance from my Teacher Mentor, as well as my own reflection on my lessons. Below are some examples of what I believe to be effective forms of both formative and summative assessment that I utilized during this internship.
Formative:
Below are some samples of exit slips that I used for both Science and Social throughout PSIII. I find that using these slips gives me excellent and instant feedback on how well students are understanding the material that they are learning. These slips really help me determine my day to day planning and what I can move on from, or what I need to spend more time on.
Also, click here to view and navigate a review game that I made to prepare students for their Science Hearing and Sound test, using a jeopardy template that I found online. It served as an excellent way for students to actively review the material from the unit, while also showing my ability to achieve KSA J, which is based on technology use and integration and can be found on this page.

Summative:
Below is the instruction sheet, planning page, marking rubric, and students samples of instruments that students created for their Hearing and Sound unit in Science. I created and sent home the instrument sheet and planning page with students before we started the project, so that they could gather materials and make a plan for their instrument. Then, students spent two days building and testing their instruments, as well as presenting them to the class. Students really seemed to enjoy the creativity they had with this assignment and overall, most instruments were really well done. This assignment showed me that students were able to create sound-producing devices that could produce different volumes and pitches. This assignment also showed students the importance of making connections between their home or community and their school, by gathering materials from outside of school and bringing them into the classroom. The collaborative nature of this project shows my ability to demonstrate KSA M, which can be found here. There are a couple of photos below that show excellent examples of student instruments.
